Specimen Collection Protocols:
Do not interchange specimen type.
Trough Level; draw sample immediately prior dose.
Peak Level; draw 30 minutes after end of IV infusion.
Give dose details, dose time, dose interval, collection time and time over which infusion is give.

Interpretation

Trough Vancomycin concentrations between 5-15 mg/L are considered to be optimal for efficiency and avoidance of toxicity. However, higher trough concentrations are recommended with some infections. Vancomycin clearance is reduced with renal impairment.
