P1NP (total procollagen type 1 N-terminal propeptide) is the preferred marker for bone formation. It is a specific indicator of type 1 collagen deposition. It is released as a trimeric structure, but degrades to a monomer. The Elecsys assay detects both, ie. total P1NP.
It is increased in states of high bone turnover and hence may be of value for assessing the response to treatment of osteoporosis and Paget's disease.
Has been reported to be a marker for bone metastases of several types of carcinoma.
