DHEA and 4AD (androstenedione) are referred to as adrenal androgens. They are secreted by the adrenals and gonads and are precursors to testosterone. There is a small diurnal pattern parallel to cortisol. Clinically, because of the association with testosterone, DHEA and 4AD are involved in the production and maintenance of secondary sex characteristics. Thus, gonadal disturbances such as delayed puberty, infertility, precocious puberty, and conditions such as Klinefelter's syndrome (XXY, primary hypogonadism) can lead to abnormal DHEA and 4AD levels.
