Diagnostic Use

Diagnosis and monitoring of monoclonal gammopathies. A monoclonal IgD paraprotein is present in 1 % of patients with myeloma, and IgD paraproteins are often present in low concentrations and do not always have a quantifiable monoclonal band on serum protein electrophoresis.
IgD may also assist with the diagnosis of Hyperimmunoglobulinaemia D syndrome (HIDS) or Hyper IgD syndrome, a disease characterised by episodes of periodic fever and other affections.

Interpretation

Quantitative analysis does not distinguish between monoclonal, oligoclonal, or polyclonal elevation. Serum protein electrophoresis and Immunofixation should be used to determine if increased concentrations are due to the presence of a monoclonal band.

IgD is normally only present in trace quantities in serum being frequently undetectable. The physiologic significance of IgD is unclear.

The presence of a monoclonal IgD is almost always indicative of a malignant plasma cell disorder, such as multiple myeloma or primary amyloidosis.
